Kvalitet av hjertelungeredning og traumebehandlingOverlevelsen etter hjertestans og alvorlige ulykker avhenger av alle ledd av redningskjeden. Ved å måle kvalitet av flere behandlingsfaktorer og resultater har vi vist forbedringspotensialer og lansert nye behandlingsmetoder og systemer.Vi har vist at det er stor forskjell på hva retningslinjene for hjerte-lunge-redning (HLR) anbefaler og hvordan både legfolk og profesjonelle reddere utfører HLR i praksis. Vi har også vist at HLR kvaliteten påvirker resultatet. Bedring av HLR kvalitet er avhengig av trening, og vi fant at folk i Oslo er mer positive til å tillate trening i vanskelige prosedyrer på nylig avdøde enn folk i New York. Hovedsakelig foregår trening ellers på dukker, og resultatet av denne var bedre hvis dukkene ga automatisk verbal tilbakemelding på kvaliteten. I samarbeid med industrien har vi innført samme automatiske tilbakemelding på HLR kvalitet via hjertestartere, og fant at dette bedret kvaliteten noe, men ikke så mye som vi hadde håpet. Det viste seg at profesjonelle reddere er 1. i stand til å gi gode brystkompresjoner, men 2. er redde for å følge vitenskapelig basert tilbakemelding fordi de tror kraftige brystkompresjoner kan gi stor skade på pasienten. Dette har ført oss inn i forskning på innføring av vitenskapelig baserte retningslinjer, et felt som er lite studert tidligere. Vi har studert hva det er mulig å oppnå av kvalitet under HLR og hvordan faktorer som brystkompresjonsmaskiner, kontroll av luftveiene, valg av ventilasjonsmåte og justeringer i bruk av hjertestarter kan bedre situasjonen. Vi har også sett på forholdet mellom kraft og dybde i brystkompresjoner på mennesker, og målt effekten på oksygentilførselen under hjertestans av forskjellige måter å ventilere på. I en dyrestudie fant vi at medikamentet adrenalin ikke hadde noen gunstig effekt hvis gitt på den måten det til daglig gis til pasienter. Et annet felt hvor ny teknologi kan bedre overlevelsen av hjertestans, er automatisk vurdering av når hjertet er best mottakelig for sjokk fra en hjertestarter. I samarbeid med forskere i Stavanger, Wien, Innsbruck og Edinburgh har vi utarbeidet matematiske metoder for analyse av hjertets elektriske aktivitet under hjertestans som kan indikere når hjertet vil begynne å slå igjen hvis vi gir et sjokk, i øyeblikket med nøyaktighet på ca 85 %. Vi har også funnet en ny metode til å oppdage feilplassering av en luftveistube i spiserøret ved å måle elektrisk motstand i brystkassen når pasienten blir pustet for. Sykehusbehandlingen etter initialt vellykket gjenopplivning er viktig for enderesultatet. Vi har vist at det er betydelige forskjeller mellom sykehus, og en systematisk gjennomført behandling med bl.a. nedkjøling av pasienten i et døgn, oppstaking av hjertets kransarterier, intens blodsukker- og blodtrykkskontroll nesten doblet overlevelsen på Ullevål US. Vi har skrevet en oversiktsartikkel om dette feltet og har vært sentrale i utarbeidelsen av internasjonale retningslinjer for rapportering av resultater fra denne behandlingen. Forskningsgruppen har også vært sentral i utarbeidelsen av de nye internasjonale retningslinjene for HLR Innenfor skadebehandlingsforskningen har vi basert på det store skaderegisteret på Ullevål US kommet med flere forslag til forbedret nøyaktigheten i det å kunne forutsi sannsynligheten for overlevelse etter alvorlige ulykker. Ved å innføre disse nye metodene blir det lettere å måle forbedringer i sykehusbehandlingen av pasienter med alvorlige skader, og lettere å sammenlikne resultatene mellom forskjellige sykehus.
