The overall aim of the Project as stated in the contract/project-description is to open up for stronger collaboration between the Group of Oncogenomics at Ahus with its clinical and basic scientists, and to best utilize the huge collection of patient materials and data generated for the benefit of the patients. By such synergism we will be able to explore genes/pathways/networks involved in basic processes like cell cycle, DNA repair, apoptosis, and immune response and their impact on breast cancer (BC) development, progression and response to therapy. By performing longitudinal studies of samples at different stages of the disease and characterize such patient materials in full molecular details, we aimed to develop more individual treatment protocols. The first objective was to understand molecular behaviors, mechanisms and relationships between and within the different types of molecular structures, including associations between these and various phenotypes, like clinical outcomes, pathways, interactions, hotspot DNA mutations and mutations in genes that drive cancer development. The second objective was to understand the taxonomy of diseases, classifying individuals (or samples) into latent classes of disease subtype, and the third objective was to predict an outcome or phenotype (like survival or efficacy of therapy) for prospective patients. Some statistical methods are specialized to one type of question; others can be used for several. Some of the tools were originally designed to reveal features of genes and pathways such as enrichment analysis, others to reveal features of patient subgroups like integrative clustering. Common to all the above mentioned approaches is that we demonstrate or map the diversity of molecular mechanisms that may lead to altered expression of a miRNA, an mRNA, or a protein. A natural extension of this was to identify and catalogue the multitude of molecular events that may lead to aberrant behaviour of a biological process or pathway (Kristensen et al PNAS, 2012). Further, we designed an algorithm to search for genomic alterations that lead to observable transcriptional perturbations both in-cis and in-trans and through these perturbations affect some process or pathway. This was applied to data from Oslo1 (MicMa) to derive a list of 56 candidate breast cancer driver genes, many of which were already known to play a role in breast cancer as well as other promising candidates that will be subject to further study within the Jebsen Centre. Finally, our previously published method of allele specific analysis of copy number variation (ASCAT) (van Loo et al 2010, 2012) is now being extended to detect subclonality. In building up the collaborative network we took into account three main considerations. First, to validate and evaluate existing models and findings and to develop novel and more advanced statistical models for breast cancer in a systematic way, we aimed to bring together a multi-disciplinary team involving basic researchers, pathologists, clinicians, bioinformaticians, and biostatisticians. Second, the biological samples and related epidemiological and clinical information are critically important for the success of the project; therefore, we have assembled together unique data collection and biorepositories combining tumour samples and blood and plasma samples obtained from large cohorts, along with large case series with extensive biospecimen and clinical information. Third, to allow genome-scale processing, analysis, and interpretation of complex and integrated enomic data, we brought together groups that have developed the pipelines for bioinformatics processing of large data sets, and biostatistics. Enabled by large accumulation of available patient material and data (DNA, RNA, protein profiling), we want to continue this research with the integration of further molecular levels towards "digital oncology" and science guided clinical decision. The aim of the project was to offer tailored and scientifically tested methods of diagnosis and therapy options for breast cancer patients as well as to continue the search for biomarkers in unexplored territories as well as increase our understanding of their function. This will reduce mortality and morbidity due to breast cancer, render health investments more efficient and help strengthen the European clinical community. By making our models widely available we aim to fill the gap between "one-for all" medicine and custom designed cancer care, the latter being by far not available everywhere (Time magazine, March 30). The choice between the one or the other depends on the available clinical trials in the hospital, the proximity to gene testing laboratory, the availability of trial slots. These factors are influenced by where a patient lives, socioeconomics, information, often pure serendipity and are of great importance particularly to patient organizations and policy makers. Comprehensive implementation of a dissemination strategy towards key target-groups and particularly to patient organizations and policy makers was undertaken.

We seek to integrate high-throughput molecular data to understand how the normal genetic background affects various parts of a biological system (e.g. metabolic pathways, organelles, cells, physiological systems, organisms etc.). The uniqueness of the project lays in the possibility to link the tumor profile to the patients genotype and exposure.The project is part of the Oslo Breast Cancer Consortium (OSBREACC) that is the fundament in the K.G. Jebsen Centre for Breast Cancer Research. In the past year two novel lines of investigation have shown rather promising and we want to pursue them with the new application: Next generation sequencing for clinical markers (using the Ion Torrent from Life Technologies and MiSeq from Illumina) and normal breast biology and risk of breast cancer. The uniqueness of the project lays in the possibility to link the tumor profile to the patients genotype and exposure and to evaluate the impact of these parameters on tumor type, aggressiveness and treatment resistance. In this we were aiming at observing, in a time specific manner, the effect of genetic variation on the molecular changes at DNA, mRNA, miRNA level in cases and extensive collection of healthy but at risk individuals. At the same time we measure the phosphorylation profiles (PamGene) at different stages of the disease progress. This basic knowledge will build a solid ground for translational research in the clinic aiming at identifying relevant biochemical pathways and individuals whose genotypes call for a particular attention in terms of modifying the disease course, aggressiveness and response to treatment. Novel aspects in this project period are the introduction of next generation sequencing experiments at both RNA and DNA level, single cell sequencing, a cutting edge technology, and the utilization of proteomics platforms such as PamGene platform for studies of protein phosphorylation activity and ER co-activator activity in biological specimen at EpiGen The utilization of the Luminex platform for thestudies of interleukin expression in plasma of breast cancer patients. The results have been published in scientific peer reviewed journals. Results informative for patients have been ommunicated in popular patient/directed journals such asAthene, forskning.no, cancer letters,and through the newsletter for The Norwegian Cancer Society.The degree of national collaboration is reflected in the participation of OSBREACC (Radium, Ullevål, Ahus, Fredrikstad, Drammen hospitals for sample acquisition, UiO and NTNU). Each subroject with the respective responsible postdoc or PhD student responsible have worked in close collaboration with coresponding from the other hospitals. In most cases postdocs and PhD students from Norway have been hosted by international collaborator, who have also in turn visited us. The performed here experiments are aiming at observing,in a time-specific manner the effect of genetic variation on the molecular changes at DNA, mRNA, miRNA level as well as the phosphorylation profiles (PamGene) at different stages of the disease progress.This basic knowledge will build a solid ground for translational research in the clinic aiming at identifying relevant biochemical athways and individualswhose genotypes call for a particular attention in terms of modifying the disease course, aggressiveness and response to treatment. Based on these stusies we started a clinical N-of-1 trial, the MetAction project, where we aim to offer approved targeted therapy to patients with metastases from solid tumors, including BC, where biopsies from the metastatic lesion has confirmed presence of the corresponding actionable target. This is the first clinical trial of this kind in Norway. The study centers are Oslo University Hospital and Akershus University Hospital.

The project supports the sample collection and preparation of the samples from Ahus group into the Oslo Breast Cancer Consortium(OSBREACC) that is the fundament in the K.G. Report operating costs: As exactly planned in the project proposal the operating costs of this project had been used as follows: 2012- SNP-CGH array profiling of the samples of the entire Oslo II series (including the Ahus part), collaboration AROS, Denmark 2013-DNA methylation profiling of the samples of the entire Oslo II series (including ther Ahus part), (postdoc 1 of this project, JK) 2013- TP53 sequencing of the Ahus part of OsloII (postdoc 1 from this project, JK) 2013. Cytokine profiling of plasma samples from OsloII and Oslo II series (PhD student from this project, SJ) The operating costs in the remainiong year will be spent 2014-DNA methylation profiling of the OsloIII series (including normal Ahus samples), collaboration CNG, Paris, France 2014-miRNA analysis of the remaining Ahus part of Oslo II (PhD student from a related HelseSørØst project, AT) Like this by the end of this year of the project we will have a comprehensive set of analyses of a set of ca 600 cases from OUS and AUS analysed at highthroughput molecular levels of DNA copy number aberrations, DNA methylation, miRNA expression and mRNA expression. Subsets of these are already subjected to integrated analyses (postdoc 2 from this project, SK) Report PhD student We have shown that by whole genome mRNA expression and CGH analyses we can identify tumors with a variety of immune responses by the spectrum of cytokines that are expressed (Kristensen et al PNAS, 2012). The main goal of this part is to identify corresponding cytokine profiles in the plasma and exploit their potential as non-invasive assay of the immune constelation of the tumor. The lab work on the first series of samples (OsloII) is finalised, the seconsd series (OsloIII is under way), data analysis of the first set is ongoing (tpogether with postdoc I and II from the project). Tumour biopsies from OsloII and III are under laboratory analyses and and will be compared to the nested case-control study within EPIC-Heidelberg (n=400 pairs), the Biological Resource Center of Gustave Roussy Institute (Paris, France, (n>300 pairs) and will include surrounding normal tissues and peripheral blood samples taken at diagnosis. This project will continue as a part of the ERA-NET on Translational Cancer Research (Transcan) project to evaluate the predictive value of individual methylation markers in circulating cell free DNA. Report postdoc 2 Data integration and next generation sequencing analysis. Using data integrating toos Paradigm and Pathifier we identify new profiles from whole tumor samples, and mirror the leukocyte complexity in the tumors. We use each gene’s Integrated Pathway Levels (IPL) to directly identify and classify the tumors according to the most deregulated pathways (across molecular data types) and then investigate the relationship of the new clusters with the previously described subclasses of breast cancer at various molecular levels. Breast cancer is the leading cause of cancer mortality in women. The prognosis of breast cancer is dependent on stage at the diagnosis, tumors diagnosed at early stage having better prognosis. It is therefore important to detect breast tumors at as early stage as possible.The following goals have been the focus of this years project: • To evaluate the gene expression profiles in preoperative tissue (core needle biopsies) while the tumor is still in its normal milieu to postoperative tissue from the same tumor obtained during surgery (Riis et al 2012A). • To find a gene expression profile which will best characterize the two most common groups of tumors: T1 and T2 tumor categories, which are the most important factors of treatment decision (Riis et al 2012B). • To use this profile for more refined selection of patients who would benefit from adjuvant treatment and would therefore be of great value to the patient preventing possible severe side effects, and to the community saving from unnecessary health expenses. • The overall goal of the K.G. Jebsen research center is to open up for stronger collaboration between its clinical and basic scientists, and to motivate exchange of ideas on how to best utilize the huge collection of patient materials and data generated for the benefit of the patients. By such synergism we will be able to explore genes/pathways/networks involved in basic processes like cell cycle, DNA repair, apoptosis, and immune response and their impact on breast cancer development, progression and response to therapy. By performing longitudinal studies of samples at different stages of the disease and characterize such patient materials in full molecular details, we aim to develop more individual treatment protocols. To explore the full spectrum of genetic changes we will also do studies on normal breast, tissue from women with high genetic risk for breast cancer and for precancerous lesions (DCIS). Ahus is a strong participant with both early stage disease and strong analytical park, including the Iontorrent, Miseq sequencing platforms as well as the PamGene station for analysis of kinase activity.
