Av funn i prosjektet er målinger av NGAL (Nøytrofilt Gelatinase-Assosiert Lipocalin) i avføringen nærmest klinisk applikasjon. Utprøving og karakterisering foregår p.t. Et stort arbeid er lagt inn i å utvikle, karakterisere og ta i forskningsmessig bruk såkalte colonoider (tykktarmsceller fra individuelle pasienter). Vi bruker nå disse for å karakterisere effekten av konkrete medikamenter, og mener at denne metoden skal kunne brukes som et element i persontilpasset medisin i ikke alt for fjern fremtid. Ellers følger forskningsgruppen en rekke forskjellige spor for å avklare sykdomsmekanismer og karakterisere sykdommen hos det enkelte individ, hvorvidt dette gir direkte følger i behandlingen må tiden vise. Prosjektets metoder for beskrivelse av pasientmaterialet og spesielt det systematiske forhold til klinisk og endoskopisk pasientevaluering har spredt seg til andre pasienter ved avdelingen som ikke er involvert i studien, og scoringskjemaer o.l. som brukes på studiepopulasjonen finnes så nyttige at de i økende grad brukes også ved vanlig poliklinisk oppfølging av pasienter med disse sykdommene. Dette en klar klinisk tilleggsverdi til selve prosjektet.
Bevilgningen er brukt til flere studier som har gitt potensiell effekt på pasientbehandling. Vi har fortsatt arbeidet med karakterisering av NGAL som markør på inflammasjon, og det pågår et langtidsprosjekt ved Dagposten, Avdeling for fordøyelses- og leversykdommer hvor vi undersøker denne markøren under pågående behandling med anti-TNF. Som ledd i forskningsprosjektet er rutinene ved Dagposten blitt forandret til en mer stringent oppfølging av behandlingsresultat, og vi har fått to forskningssykepleiere (samlet 50% stilling) over de kommende to årene til å følge og karakterisere pasientene nøye. Dette har sannsynlig positiv langtidseffekt for denne pasientgruppen. Det er videre lagt ned et stort arbeid i å etablere pasientspesifikke ex vivo modeller (organoider) som er et godt utgangspunkt for presisjonsmedisinske tiltak overfor gruppen.
Arbeidet dreier seg i stor grad om å forstå sykdomsmekanismer og dreie resultatene over i klinisk nytte. Det mest konkrete er at vi har etablert NGAL som en sikker markør for betennelse i tarm, og skal videreføre dette på et prospektivt klinisk materiale gjennom Dagposten ved Avdeling for fordøyelses- og leversykdommer, St. Olavs Hospital. Videre har vi isolert ut lovende molekylære markører for inaktiv betennelse som kan bli av diagnostisk betydning. Kommende år vil vi utnytte våre funn så langt direkte i et prospektivt klinsk materiale.
