2009

NevrologiForskning på inflammatoriske og genetiske nevrologiske sykdommer foregår i hovedsak ved Nevrologisk Forskningslaboratorium. I tillegg foregår det utstrakt klinisk nevrologisk forskning.Ved Nevrologisk Forskningslaboratorium er de største forskningsfeltene konsentrert om multippel sklerose, paraneoplastisk nevrologisk sykdom, myasthenia gravis og mitokondriesykdommer. Videre foregår det utstrakt klinisk forskning ved Nevrologisk avdeling, spesielt innen multippel sklerose, hjerneinfarkt, nevrorehabilitering, Parkinson sykdom og epilepsi. Nevrologisk Forskningslaboratorium har nært samarbeid med Nevro-Revma-laboratoriet, LKB innen nevroimmunologi diagnostikk. Dette gjelder spesielt for utredning av multippel sklerose, myasthenia gravis, nevropati og paraneoplastiske nevrologiske sykdommer. Det er tilknyttet biobanker til Nevrologisk Forskningslaboratorium. Ved laboratoriet foregår det hovedsakelig grunnforskning innen nevroimmunologi og genetikk på humant materiale. Dessuten er det etablert to dyremodeller for multippel sklerose og mitokodriesykdom. Strategiske forskningmidler er fra 2004-2008 tilført Nevrologi-miljøet ved Haukeland Universitetssykehus. Resterende midler har vært overført til 2009. Disse er blitt brukt til å ferdigstille prosjekter innen multippel sklerose, hjerneslag, paraneoplasi, myasthenia gravis og mitokondriesykdommer. Midlene har i hovedsak vært brukt til reagenser ved Nevrologisk Forskninglaboratorium. Det ble i år avlagt en doktorgrad innen hjerneslag ved Nevrologisk avdeling, Haukeland Universitetssykehus. Det er også flere stipendiater her tilknyttet multippel sklerose, paraneoplasi, myasthenia gravis og mitokondrie forskning.

2008

Nevrologi (NFR-evaluering)Forskning på inflammatoriske og genetiske nevrologiske sykdommer foregår i hovedsak ved Nevrologisk Forskningslaboratorium. Forskningen er pasientrelatert og har medført at en rekke immunologiske og genetiske anlyser er innført ved HUS. Videre foregår det utstrakt klinisk forskning.Vår forskning er translasjonell med et nært samarbeid mellom laboratorierettet / klinisk forskning og pasient diagnostikk / behandling. Innen disse områdene er de største forskningsfeltene konsentrert om: Inflammatorisk hjernesykdom, dvs multippel sklerose og paraneoplastisk nevrologisk syndrom; og nevromuskulære sykdommer, dvs nevropati, myasthenia gravis og myopati. Videre foregår det utstrakt klinisk forskning ved Nevrologisk avdeling, spesielt innen multippel sklerose, hjerneinfarkt, nevrorehabilitering, parkinson sykdom og epilepsi. Den immunologiske forskning er konsentrert om påvisning og karakterisering av ulike autoantistoffer ved multippel sklerose, paraneoplastiske syndromer, nevropati og myasthenia gravis. Basert på slik forskning er det innført ulike laboratorietester som brukes til diagnostikk. Dette gjelder antistoffer mot paraneoplastiske antigener, gangliosid, glykoprotein, interferon-beta, natalizumab, titin, AChR og VGCC. Dette er analyser som utføres ved HUS. Det er et nært samarbeid med University of Oxford hvor også andre spesialanalyser kan utføres (bl.a. antistoffer mot VGKC og aquaporin-4). Den genetiske forskning er i hovedsak rettet mot ulike kandidatgener ved multippel sklerose og påvisning av gener som kan disponere for myopati. Vår forskning har tidligere medført at duplikasjon/delesjon av PMP22 genet i forbindelse med diagnostikk av arvelig nevropati er innført ved HUS. Nyere resultater ved Nevrologisk forskningslaboratorium har også ført til at mutasjons-analyser av POLG genet i forbindelse med mitokondrie diagnostikk er innført ved HUS. Det er i år etablert en dyremodell for multippel sklerose hvor ernæringsforsøk gir lovede resultater. Det er også utstrakt klinisk forskning ved Nevrologisk avdeling. Det er opprettet pasientregister innen hjerneinfarkt. Innen multippel sklerose er det også et nasjonalt pasientregister, samt at det nylig er etablert en nasjonal biobank. Det vises forøvrig til separate prosjektbeskrivelser. Det ble i år avlagt tre doktorgrader innen multippel sklerose og en innen nevrofysiologi (epilepsi).

2007

Inflammatoriske og genetiske nevrologiske sykdommerDet er et nært samarbeid mellom Nevrologisk Forskningslaboratorium og klinisk nevrologisk forskning. Forskningen er spesielt konsentrert omkring inflammatoriske og genetiske nevrologiske sykdommer. Det drives også utstrakt klinisk forskning innen bl.a multippel sklerose, hjerneinfarkt og nevrorehabilitering.Forskning ved Nevrologisk avdeling, Haukeland Universitetssykehus er spesielt fokusert på immunologi og genetikk. Innen disse områdene er de største forskningsfeltene konsentrert om: 1. Infammatorisk hjernesykdom, dvs multippel sklerose og paraneoplastisk nevrologisk sykdom. 2. Nevromuskulære sykdommer, dvs nevropati, myasthenia gravis og myopati (mitokondriesykdommer). Videre foregår det utstrakt klinisk forskning ved avdelingen, spesielt innen: 1. Multippel sklerose. 2. Hjerneinfarkt. 3. Nevrorehabilitering. Den immunologiske og genetiske forskning er lokalisert til Nevrologisk Forskningslaboratorium. Den immunologiske forskning er konsentrert om humorale immunologiske reaksjoner, dvs i hovedsak karakterisering av autoantistoffer, spesielt ved multippel sklerose, paraneoplastiske nevrologiske sykdommer og myasthenia gravis. Den genetiske forskning involverer bl.a. karakterisering HLA og andre kandidatgener ved multippel sklerose, mikromatrise studier av kreftceller behandlet med paraneoplastiske antistoffer og mitokondrie funksjoner. Det er påvist mutasjoner i POLG genet som årsak til mitokondriesykdommer. Dette feltet har nå fått midler fra Forskningsrådet. Screening av ulike autoantistoffer er innført som rutine ved Nevro-Revmalaboratoriet, LKB, og screening for POLG mutasjoner er innført som rutine ved Avd. for Medisinsk Genetikk. Ved Nevrologisk avdeling drives det også utstrakt klinisk forskning. Dette gjelder bl.a. multippel sklerose og nevrorehabilitering. Innen feltet hjerneinfarkt ble det fullført doktorgrad.

2006

Nevromuskulære sykdommer, dvs nevropati, myasthenia gravis og myopati (mitokondriesykdommer). Immungenetisk forskning foregår i hovedsak ved Nevrologisk Forskningslaboratorium. Den immunologiske forskning er konsentrert om påvisning og karakterisering av ulike antistoffer ved multippel sklerose, paraneoplastiske sykdommer og myasthenia gravis. Basert på slik forskning er det innført ulike laboratorietester som brukes til diagnostikk. Det er utviklet sensitive metoder for å måle antistoffer mot interferon. Interferon-behandling brukes ved multippel sklerose, og påvisning av slike antistoffer kan resultere i terapisvikt. Interferon og andre cytokiner's effekt studeres på leukocytter fra pasienter med multippel sklerose. Ved paraneopastiske nevrologiske sykdommer er det påvist nye autoantistoffer som karakteriseres. Antistoffenes relasjon til ulike kreftformer og nevrologiske sykdomsbilder studeres. Bindingsevnen varierer for ulike auto-antistoffer som derved kan påvirke deres effekt. Videre studeres autoantistoffenes effekt på kreftceller i kultur. Dette gjøre vha genekspresjonsstudier, som også gjøres for å karakterisere multippel sklerose lesjoner. Den funksjonelle aktivitet til antistoffer som er relatert til myasthenia gravis studeres på muskelceller i kultur. Proteomikk studier er startet opp for å finne biomarkører i spnalvæske fra pasienter med multippel sklerose. Utstrakt forskning er også rettet mot mitokondrienes funksjon. Det er påvist at mutasjoner i polymerase genet POLG er en viktig årsak til mitokondrie-sykdommer, og slik diagnostikk kan nå tilbys ved sykehuset. Kliniske korrelasjoner til slike mutasjoner studeres, og det skal lages dyremodell for nærmere karakterisering av disse genenes funksjon.

2005

Den er spesielt konsentrert omkring inflammatoriske og genetiske nevrologiske sykdommer.Forskning ved Nevrologisk avdeling, Haukeland Universitetssykehus foregår i tett samarbeid med Seksjon for nevrologi, Institutt for klinisk medisin ved Det medisinske fakultet, Universitetet i Bergen. Forskningen inngår i bl.a. Locus for nevrovitenskap og Locus for resisterepidemiologi, samt er en del av Forskerskolen for klinisk medisin. Forskningen er translasjonell med et nært samarbeid mellom laboratorierettet og klinisk forskning. Den er spesielt fokusert på immunologi, genetikk og register-epidemiologi. Innen disse områdene er de største forskningsfeltene konsentrert om: - Inflammatorisk og iskemisk hjernesykdom, dvs multippel sklerose, paraneoplastisk sykdom og hjerneinfarkt. Innen dette feltet ble det i 2005 avlagt to doktorgrader som omhandlet paraneoplastisk nevrologisk sykdom og akutt hjerneinfarkt. -Nevromuskulære sykdommer, dvs nevropati,myasthenia gravis og myopati. Immunologisk og genetisk forskning foregår i hovedsak ved Nevrologisk Forsknings-laboratorium. Den immunologiske forskning er konsentrert om påvisning og karakterisering av ulike antistoffer ved multippel sklerose, paraneoplastisk sykdom og myasthenia gravis. Basert på slik forskning er det innført ulike laboratorietester som brukes til diagnostikk. Den genetiske forskning er i hovedsak rettet mot ulike kandidatgener ved multippel sklerose og påvisning av gener som kan disponere for nevromuskulære sykdommer. I 2005 ble det fra forskningsgruppen påvist en ny mutasjon i genet for mitokondriell polymerase som årsak til mitokondriesykdom. Den laboratorierettede forskning er nært knyttet til klinisk og epidemiologisk forskning. Sistnevnte er spesielt koblet mot ulike pasientregistre. Forskningsgruppen har høy vitenskapelig produksjon og har et bredt nasjonalt og internasjonalt nettverk, deriblant flere EU prosjekter

2004

Nevroimmunologi/genetikkNevrologisk avdeling fikk meget god evaluering av Forskningsrådet innen nevroimmunologi/ genetikk. Denne forskning innbefatter i hovesak laboratoriebaserte studier av multippel sklerose, myasthenia gravis, paraneoplastiske nevrologiske sykdommer og mitokondrie-sykdommer.Forskningsmidlene ble tildelt sent i 2004 og er i hovedsak overført til 2005. Midler har medgått til videre immun-genetiske studier: Multippel sklerose: Her arbeides det bl.a. med immuologiske reseptorer for IgG (FcR). Det finnes ulike genotyper som er karakterisert og vist å være assosiert med sykdomsprognose. FcR ekspresjon på leukocytter studeres i sammenheng med ulike immun-modulerende behandlingsformer. IFN-beta brukes bl.a. som immun-modulerende behandling. Noen pasienter utvikler neutraliserende antistoffer, og assay for dette er utviklet og planlegges tatt opp som rutinediagnostikk. Videre studeres immun-patologiske forandringer i hjernevev og det er vist at bla cortical demyelinisering er utbredt ved multippel sklerose. Myasthenia gravis: Antistoffer mot AChR, titin og ryanodin-reseptor er korrelert til sykdom og deres funksjonelle egenskaper studeres. Det er etablert muskel-celler i kultur og antistoffene påvirker cellenes viabilitet. Videre karakterisering av dette pågår. Paraneoplastiske sykdommer: Ulike autoantistoffer er assosiert med cancer og kan påvises serologisk. Det er utviklet ny assay for dette. Antistoffene kan også påvises i ryggmargsvæske. Nye paraneoplastiske antistoffer er karakterisert. Antistoffenes funksjonelle egenskaper studeres, samt egenskaper i tumor som kan disponere for utvikling av paraneoplastisk sykdom. Mitokondriesykdommer: Nye mitokodrie-assosierte mutasjoner er påvist. Dette gjelder bla. defekt i et kjerne-gen (polymerase gamma). Dette kan brukes diagnostisk og skal videreføres i funksjonelle studier. Det vises forøvrig til egne rapporter vedrørende de ulike prosjektene.
