Economic evaluation of using telemedicine in a direct patient care

The main aim is to contribute to the telemedicine research field through three distinct approaches: investigating the efficiency potential and the health outcomes of treatment and advice via telemedicine technologies in two randomised controlled effect studies. The first three papers were founded on two intervention studies of novel telemedicine services. In the first effect study (Paper I), we found that offering patients electronic access to their GPs via a secure messaging system significantly reduced the number of office visits. The second effect study (Paper II) found the eczema counselling service of no effect. The parents in both groups performed fewer treatments per week and they reduced their total visits to their health care providers. We also found a positive correlation between emergency visits at baseline and messages sent. In the outcome evaluation study (Paper III), the group of patients with electronic access valued it lower than the group without such access suggesting it was less useful than expected. And finally, the literature search identified 33 articles describing economic evaluations of telemedicine services; only less than one third of these articles may be considered as rigorous evaluations. Economic evaluation of telemedicine is not in accordance with accepted methodological standards for economic evaluation.
The main contribution of this work is that is shows that introducing web-messaging systems in primary and secondary care have the potential to improve efficiency in health clinics. Using messaging system for clinical communication between patients and their GPs does offset office visits by approximately one visit per patient per year. This work also shows that about half of those given access to the system actually use. The patients sent on average 1.5 messages per year and 3 messages per year for the parents, implying that concerns about overuse and too many messages may be unfounded. More research is, however, needed to establish if messaging systems are cost effective for the health care system as a whole. This work also shows that eliciting patient preferences alongside a randomised controlled trial using the WTP-method gives valuable information on the relative strengths of patient preferences. And finally, this thesis supports the notion that the quality of economic evaluations of telemedicine is not in accordance with accepted methodological standards for economic evaluation. Less than one third of these studies were rigorous economic evaluations with reliable evidence for decision-making. Economic evaluation studies as basis for decision making in telemedicine must therefore be used with care.
Investering i telemedisinke tjenester har blitt gjort med en forventning om at pasientene får samme helsetilbud som ved en vanlig konsultasjon eller sykehusbesøk og at de sparte reisekostnadene mer enn oppveier for kostnadene. Det er gjort lite forskning på feltet og de få økonomiske studiene som er gjort viser til blandede resultater.Dette prosjektet skal bidra med informasjon og kunnskap om hvordan økonomiske analyser av telemedisin i praksis kan og bør utføres. En utfordring med økonomiske studier er å finne en god balanse mellom intern validitet og generaliserbarhet. Resultater fra dette litteraturstudiet viser at det finnes få økonomiske analyser som både måler kostnader og helseeffekter av å benytte telemedisin og av disse igjen var kun et fåtall av god kvalitet. (artikkel 4)
I tillegg er det i avhandlingen inkludert to case-studier (RCTer) hvor effektene av å tilby elektroniske konsultasjoner er kartlagt. Det første studiet undersøkte om elektronisk kommunikasjon mellom fastlege og pasient erstattet vanlige konsultasjoner og telefoner. (artikkel 1). Det andre studiet analyserte effektene av å tilby elektronisk veiledning til foreldre til barn med atopisk eksem (artikkel 2).
En tredje artikkel har kartlagt hvordan pasientene verdsatte det å ha tilgang til legen elektronisk ved hjelp av en metode kalt ’betinget verdsetting’. Vi fant her litt overraskende, at de pasientene som ikke hadde tilgang til systemet dvs. de i kontrollgruppen verdsatte mulighet for å sende en elektronisk henvendelse til fastlegen høyere enn de som faktisk hadde slik tilgang (artikkel 3).
